Membership Cancellation Policy

You may cancel your Bloom360 membership at any time through one of the following methods:

  • Account Settings: Log in to your Bloom360 account and navigate to your membership settings to initiate cancellation. You can cancel online without talking to anyone, and without any additional step we do not also require to sign up.
  • Email: Send a cancellation request to care@bloom360.com from the email address associated with your account.
  • Phone: Call us at (833) 348-3044.

You will receive a confirmation of your cancellation request within two (2) business days. If you do not receive a confirmation, please contact us to ensure your request was received.

If you signed up through an employer or group arrangement, cancellation may be handled by that sponsor. Contact us and we will tell you which applies to you.

Cancellation is effective at the end of your current billing cycle. You will continue to have full access to all membership benefits, including scheduled telehealth visits, asynchronous messaging with your care team, care coordination, and health records, until the end of the period you have already paid for. After cancellation, your membership will not renew and you will not be charged for subsequent periods.

To avoid being charged for the next billing cycle, submit your cancellation request before your next renewal date. For annual memberships, we will send you a renewal reminder at least fifteen (15) days and no more than forty-five (45) days before your renewal date. That reminder will state the renewal date, the length of the renewal term, the amount you will be charged, how often you will be charged, and how to cancel.

Regular membership charges will continue through the end of your current billing cycle. You will not be charged for any billing periods that begin after your cancellation takes effect. Cancelling does not erase amounts you already owe for services already provided.

Monthly memberships

Bloom360 does not provide prorated refunds for partial billing periods. If you cancel mid-cycle, you will retain access to your membership benefits for the remainder of that billing period, but no refund will be issued for the unused portion.

If you are enrolled in a couple membership, either member may cancel their individual participation without affecting the other member’s enrollment. Upon one member’s cancellation, the remaining member’s plan will transition to the individual membership rate at the start of the next billing cycle following the cancellation effective date. We will notify the remaining member of the new rate before it takes effect, and that member may cancel instead if they would rather not continue at the individual rate. If the couple membership was paid annually, we will apply the annual refund approach in Section 2 to the departing member’s share.

Bloom360 may suspend or terminate your membership with or without cause. Where practicable, we will provide at least thirty (30) days’ advance notice before cancellation. We may suspend or terminate your membership immediately, without prior notice, if we determine in our reasonable discretion that:

  • Safety Concerns: Your continued use poses a risk to patient safety or the safety of our staff.
  • Fraud: You have engaged in fraud or misrepresentation, including providing false identity or health information.
  • Non-Payment: You have failed to pay membership fees when due. We will email you and give you at least ten (10) days to fix a failed payment before we terminate for non-payment.
  • Violation of Terms: You have materially violated our Terms of Service, including prohibited conduct.

If Bloom360 cancels your membership, we will use reasonable efforts to (a) address any urgent healthcare needs during a transition period, (b) make your health records available to you or a provider of your choice, and (c) provide information to help you find alternative care.

After your membership ends, your health records will be retained in accordance with applicable state and federal law, which generally requires retention for a minimum of seven (7) years from the date of your last encounter. You may request copies of your health records at any time by contacting us at care@bloom360.com. We will process your request in accordance with applicable law and may charge a reasonable fee for copies as permitted. Any fee will be limited to the cost-based amount permitted under 45 CFR 164.524, and we will respond within thirty (30) days. In practice, an electronic copy sent to you or your next provider is free.

If you cancel your membership and later wish to rejoin, you are welcome back. Rejoining members will be subject to the current membership pricing at the time of re-enrollment. We do not charge a re-enrollment penalty, but founding or promotional rates are not guaranteed to still be available. Your previous health records will be available to your care team (subject to our retention policies), which helps ensure continuity of care.

Loose ends are the part people actually worry about, so here is our commitment:

  • Prescriptions: Active prescriptions already sent to your pharmacy remain valid until they expire or run out. We will not issue new prescriptions or refills after your membership ends, so line up a new prescriber before your last day.
  • Labs and imaging already ordered: If a Provider ordered it before your membership ended, we will review the result and communicate it to you even if it comes back afterward. We will not leave a pending result unread.
  • Referrals and open messages: We will close out anything in flight and tell you what is still outstanding.
  • Third-party costs: Charges billed directly to you by a lab, pharmacy, imaging center, or other outside provider are between you and them. Cancelling your membership does not cancel those.
